Please fill out the form below to receive a personalized unlimited reuse approval. You may purchase one quantity of the approval per time as often as is needed for future orders. We screen combinations that are not recommended by our in house doctor or exceed a 90 day supply per order.

We will respond within 24 hours and you may check out with the link in the email containing the products. 

If you feel our approval screened incorrectly, but your current physician approves of the combination or dose of medicines then please send a reply to reconsider the request.
